"Tuttle Dictionary of the Martial Arts of Korea, China & Japan"
by Sun-Jin Kim (Contributor), Daniel Kogan (Compiler), Nikolaos Kontogiannis (Compiler), Hali Wong (Contributor)

This book may not meet all of your criteria, but I've found it helpful. You can find it here ...

If you don't mind wadding through the Japanese or already proficient in the language, then I would recommend:

Watatani K. & Yamada C. (1978). Bugei Ryuha Dai Jiten (An Encyclopedia of Martial Arts Ryuha). Tokyo: Tokyo Copy Co.

It retails for about $100.00 US, a little on the pricey side.
